Parent and Guardian Consent Agreement

Effective Date: July 2026

By registering a student for Empowered Pathways, the parent or legal guardian acknowledges and agrees to the following terms.

Authority to Consent

I confirm that I am the student’s parent or legal guardian, or that I otherwise have legal authority to consent to the student’s participation.

I authorize the student identified during registration to participate in the selected Empowered Pathways cohort and related program activities.

Program Participation

I understand that Empowered Pathways is a 12-week educational and personal development program for teens ages 14 to 18.

Program activities may include:

Group discussions and collaborative activities
Self-discovery and strengths assessments
Leadership and communication practice
Emotional awareness and self-regulation activities
Creative exercises and reflective workbook activities
Career, education, financial awareness, and entrepreneurial exploration
Student presentations and a final showcase

I understand that the program may be delivered virtually through platforms such as Zoom or in person at the location identified during registration.

Educational Scope

I understand that Empowered Pathways provides educational and personal development programming. It does not provide licensed therapy, psychological counseling, medical treatment, legal advice, individualized financial or investment advice, or accredited academic instruction.

I understand that individual experiences and outcomes will vary. No specific academic, emotional, financial, entrepreneurial, or personal result is guaranteed.

Student Information and Support Needs

I agree to provide accurate and current registration, emergency contact, allergy, health, accessibility, and support information that may reasonably affect the student’s safe and successful participation.

I am responsible for notifying Empowered Pathways if relevant information changes during the program.

Participation and Conduct

I understand that students are expected to participate respectfully and follow reasonable program, technology, facility, and group-safety expectations.

Empowered Pathways may contact me regarding participation, behavioral, safety, or support concerns.

Serious or repeated conduct that is disruptive, unsafe, threatening, inappropriate, or harmful to the group environment may result in removal from the program, subject to the Refund Policy and applicable law.

Virtual Session Recordings

I understand that virtual sessions may be recorded for private educational review, missed-session support, program administration, safety, and recordkeeping.

I understand that the student’s name, voice, image, participation, presentation, or work may appear in these recordings.

Private educational recordings may be shared through private methods with enrolled families and authorized program personnel. They will not be posted publicly or used for advertising unless separate promotional permission has been granted.

The student will be informed when recording is taking place and will be required to acknowledge the recording before participating in a recorded session.

Public Promotional Permission

I understand that public use of the student’s image, voice, work, photograph, video, or presentation is addressed through a separate permission choice during registration.

Declining public promotional permission will not affect the student’s eligibility, enrollment, participation, or access to the program.

Technology and Online Participation

For virtual participation, I understand that I am responsible for providing suitable internet access, a compatible device, and an appropriate location for the student to participate.

I understand that online platforms and third-party services may have their own terms, privacy practices, and technical limitations.

Electronic Agreement

By entering my name, selecting the required acknowledgments, and submitting the registration form, I intend to sign this agreement electronically.

I understand that my electronic submission has the same effect as my handwritten signature to the extent permitted by applicable law.
